To request time served, you must provide proof of incarceration from a jail or prison that includes the dates you were incarcerated, along with the request form. To consider your request, you must enter a plea for the charges for which you are requesting jail credit, if you have not done so already. You may also submit your request in person or by mail. Your request is not granted unless the judge approves your request.

PLEASE NOTE: Submission of this request will result in a conviction. Failure to comply with an order granted by the judge will require you to appear at a Show Cause Hearing before the Municipal Court judge.
